The church was designed by the architect and canon Rosato Rosati to the same design as the church of San Carlo ai Catinari in Rome and according to the usual layout of Jesuit churches: a single, very wide nave with deep side chapels, an imposing dome with a very high lantern, and a bell tower. It was completed in 1625, after Rosati's death.

It had a strategic location: it served as the entrance to the city center for those coming from that direction. As one of the gates in the city wall that once enclosed the historic center of Macerata, Porta Picena had the typical function of access routes: traffic regulation, defense, and marking the boundary between the inside and the outside of the city.

The church, which was elevated to the rank of cathedral in 1320, underwent several reconstructions over the centuries. The present neoclassical structure is the result of a reconstruction that began in 1771, following a project by the architect Cosimo Morelli, and was completed in 1790.

The origins date back to 1447, when the local community, to implore the end of a devastating plague epidemic, built a small votive temple dedicated to the Virgin Mary in a single day. Between 1736 and 1741, the building underwent a significant transformation, giving the basilica its current baroque appearance. The interior is adorned with frescoes by Francesco Mancini, including the Assumption of Mary on the ceiling, and works by Sebastiano Conca, such as the Immaculate Conception and the Nativity of Mary.

The Palazzo Buonaccorsi houses the civic museums of Macerata, ranging from ancient to modern art. Among the treasures on display are paintings, sculptures, antique furniture, and an exceptional collection of carriages. The true jewel of the palace, however, is the Galleria dell’Eneide (Gallery of the Aeneid), a masterpiece of baroque decoration, created between 1710 and 1720.

The Sferisterio of Macerata is located directly on the city wall. The semi-elliptical arena was originally intended for the game 'Pallone col Bracciale', popular in Central Italy in the 18th and 19th centuries, and for various other events. In the 20th century, it was increasingly used as a concert and opera house (the only open-air theatre in the region with boxes).
